A Note from Gaylea McDougal, CPM-TN

               

Good news! After 5 years of hands on, traditional training with Kathy Williams, CPM-TN, I have finally completed all my NARM requirements, passed my state licensing exam and am officially a Licensed Certified Professional Midwife!

I am currently taking healthy, low-risk women within a 30 mile radius of Gallatin (37066). I am currently booked through July 2017 so please contact me ASAP to set up an interview as the calendars of homebirth midwives tend to fill up fast. I will taking off the month of December so I can take clients with due dates ranging from August-(early) November at this time. I would be happy to sit down with you and answer your questions, discuss your options and dispel any myths regarding prenatal care and delivery in the home setting.

 

YOUR INVESTMENT INCLUDES:

Initial visit between 10-12 weeks (head to toe physical assessment, labs including a complete OB panel and blood typing, detailed medical and psychological history, urinalysis, vital sign assessment, auscultation of fetal heart rate and a general plan for delivery)

Monthly *prenatal visits until 28/30 weeks

*Prenatal visits include (at minimum) urinalysis, assessment of fetal movement, maternal physical and psychosocial well-being, diet/exercise consultation, maternal weight assessment, Blood Pressure assessment, fetal heart rate auscultation with doppler, fundal height measurement and Leopold's maneuver to assess fetal position

Bi-monthly prenatal visits from 28-36 weeks

Weekly prenatal visits from 36 weeks until delivery

Hemoglobin and Glucose testing at 28/30weeks

GBS testing at 36 weeks

Attendance at the onset of active labor until 2-3 hours after delivery by me and another CPM/assistant (the assistant's fee is already included in this price)

Newborn assessment (weight, height, head & chest measurements and head to toe physical assessment)

Neonatal Resuscitation if necessary (I am NRP certified by the American Academy of Pediatrics and will have all neonatal resuscitation equipment including oxygen available if necessary)

Maternal hemorrhagic assessment and treatment if necessary 

Administration of Vitamin K and erythromycin to baby if you choose to have these newborn interventions

Clean up of birth setting and laundry

Prepare your first meal if necessary after you deliver

Tennessee PKU screening, maternal health/lactation assessment & weight check between 48-72 hours post-partum 

6 week post-partum assessment of mother and baby and discharge if appropriate

 

 

 OPTIONAL SERVICES (additional fees):

*Ultrasound referral to Roy Chamberlain, R.D.M.S at 20 weeks for gender (optional) and anatomy scan. Ultrasound is an additional $100.00. 

 

- Rhogam injection if you have a negative blood type and your baby has a positive blood      type. Typing of baby will be done at delivery. Rhogam is an additional $150.00

- GBS treatment during labor (IV or PO antibiotics)- prices vary based on treatment

- IV therapy during labor for dehydration or excessive bleeding

- Additional labs or testing will be itemized individually

- Newborn medications (Vitamin K and Erythromycin). If you desire to have these medications adminsitered to your baby you will need to purchase them before the time of delivery. I can give you resources on where to purchase.  

- Placenta encapsulation- Performed by one of our Roots providers: $200 (capsules only), $225 (Capsules and tincture)

*If you are a VBAC, an ultrasound is necessary to determine placenta placement for proper risk assesment. 

INVESTMENT:

My fee for all the above services is $3500.00

 

I accept cash, check or credit/debit card (including HSA cards). I do NOT file with insurance but strongly encourage you to file on your own behalf after delivery. Many insurance companies will tell you that they cover homebirth. However, you should ask specifically about the CPM credential. Many only cover the CNM credential which is typically a hospital/birth center midwife; not a homebirth midwife. 

A deposit of $500.00 is required to secure your EDD on my calendar and must be paid before the initial prenatal visit.

Balances must be paid before 36 weeks to ensure adequate coverage for your delivery.

 

RISK ASSESSMENT:

The Midwifery Model of Care™ is based on the fact that pregnancy and birth are normal life events. The Midwifery Model of Care includes:

 

•           monitoring the physical, psychological and social well-being of the mother and consistently monitoring fetal growth

 

•           providing the mother with individualized education, counseling, and prenatal care

 

•           continuous hands-on assistance during labor and delivery and postpartum support

 

•           minimizing technological interventions 

 

•           identifying and referring out women who require obstetrical attention

 

 

The responsible application of this model has been proven to reduce the incidence of birth injury, trauma, and cesarean section. In order for this model to continue having above average satisfactory outcomes, it is important that the midwife is diligent in properly assessing criteria to ensure each client is an appropriate candidate for homebirth. I will discuss your history and any possible risk factors with you in detail at your first visit and we will check in periodically during your prenatal period to ensure your risk category has not changed. In the event that you risk out in 3 or more areas (based on a risk assessment that you willl review), I will (in most cases) recommend a transfer of care to a higher level obstetrical practitioner or delivery facility.
